Completing and Signing Digital Forms
The MPR form is a fillable PDF. In order to complete and sign the MPR form, you will need to have Adobe Acrobat or similar software installed on your computer. All PCs on campus should have Adobe Acrobat installed. If your PC on campus does not have Adobe, contact helpdesk@baruch.cuny.edu to get Adobe Acrobat installed.
If you are using a laptop or a personal desktop, the college also provides you with Adobe Acrobat. To install Adobe Acrobat on your laptop, contact helpdesk@baruch.cuny.edu.
Once you have Adobe Acrobat on your computer, you will be able to fill in the fillable MPR form. When you have filled it in, make sure you save it to your desktop or other location on your computer (for administrative convenience, please name your file in accordance with the naming protocol specified by your department chair).
